Tired of moving your hand on the mouse? 3D Virtua may have an answer for you, although it will cost around $500:
NaturalPoint™ trackIR™ mouse alternative products give you hands free control of a computer cursor. The ergonomic trackIR™ provides precise cursor control through simple head movement allowing your hands to remain on your keyboard, or at your side.
If you're looking for a productivity boosting hands free cursor control option that helps you avoid problems like carpel tunnel and repetitive strain injury, then the trackIR™ is your ideal solution. The trackIR™ is the world's only true ergonomic computer mouse because unlike other ergonomic mice, the trackIR™ is totally hands free.
Unless the sensor is extremely sensitive, I can see this causing neck problems. More information on the product here.

Hmmm... I wonder what other "useful" purpose this hands-free mouse would serve?

I used a mac version of this during rehab 6+ years ago. The one I used had a headset instead of a dot. It also had a straw on the end which you puffed on to click. Very cool, but the headset left dents in my head.

in spite of its cute little blurb, it would be mainly for people with severe neuromuscular disorders. which is a good thing. but carpal tunnel sufferers would find it too unwieldly to use regularly.

I wonder what happened to the "Mental Mouse" that was in the works. I got to use one at a computer tech show about a year and a half ago. That thing was awesome, but it had some things to be worked out still.
It consisted of a band that wrapped around you head and 2 things that went over your index and middle fingers that looked somewhat like a splint you would wear if you had a broken finger(very thin though). You didnt have to move your head, all you had to do was think about where you wanted the mouse to go, and it would move there. To click, you squezed your thumb and index finger together for left click and your thumb and middle finger for right click. There was also a little dial on the side of the index finger one that was a scroller.
They had a "mental keyboard" there too but it wasnt working right so it was just displayed with a video.

It is very useful, but like all other cool stuff, $500 is a bit pricey. Also, I think the "dot" it uses would annoy the hell out of me. I hate having stuff stuck on my face.
On a similar note, I got Dragon Dictate on my old Aptiva (pos) and the whole suite was $700....... now it's like $100. Here's hoping that they come out with a mental mouse, so I can afford the hands-free. :P
